The Adulthood Planning Consultation is a person-centered planning session that maps where someone is now and what comes next, then turns it into a clear, usable plan.
Two ways to take part
Which track fits is based on how a person can participate, not their age.
Self-Directed: The client takes part directly, shares their own goals, and signs for themselves, with caregivers supporting alongside.
Caregiver-Supported: A caregiver signs and informs, while the client takes part as fully as they can, using AAC or supported decision-making. A good fit for minors, communication differences, or higher support needs.
What you receive
A written, person-centered plan you can act on and share
A prioritized list of the goals and supports that matter most
Referrals and clear next steps, including a path into ongoing support if you want it

The consultation is the planning step:
If you decide to continue into ABA, an Initial Assessment then sets a measured baseline, and skill-building begins from there. Many families use the consultation on its own, as a roadmap for navigating other community supports.
